Hormones and autism. Why our cycles matter more than we realise

Many autistic girls and women grow up feeling that how well they can manage day to day life changes from week to week, yet they are rarely told that hormones may be playing a significant role.

Oestrogen, progesterone, and testosterone do far more than regulate periods or fertility. These hormones act directly on the brain. They influence neurotransmitters such as dopamine and serotonin, which support attention, mood, emotional regulation, and sensory processing (Hampson, 2018). In other words, the very systems many autistic people are already working hard to manage.

Across the menstrual cycle, rising oestrogen is often associated with better focus, sociability, and cognitive flexibility. When oestrogen drops, particularly in the luteal phase before a period, many women notice increased overwhelm, sensory sensitivity, fatigue, shutdown, or meltdowns. Tasks that felt manageable even days before can suddenly feel exhausting or impossible. These changes are linked to measurable differences in brain activity and emotional processing across the cycle (Dubol et al., 2021).

For autistic girls and women, these hormonal changes can have an even greater impact on emotional wellbeing and day to day functioning. Masking can take far more effort, communication may feel harder, and tolerance for noise, touch, or sensory input can reduce significantly. For many, this is not mild or manageable, it can become severe enough to meet the criteria for PMDD (pre-menstrual dysphoric disorder), with profound changes in mood, anxiety, and irritability that affect work, relationships, and everyday life. This is reflected in the research, with studies suggesting that autistic girls and women report higher rates of significant menstrual related difficulties and PMDD than non-autistic peers (Moseley et al., 2020).

This is not a lack of effort or willpower, it is neurobiology.

Understanding how your hormones affect you each month can be empowering. Tracking your cycle, planning more demanding tasks for higher energy days, allowing space for slower days before your period, and seeking appropriate medical support can reduce self-blame and help you work with your body rather than against it. When we recognise that our brains and hormones are inseparable, we can respond with greater compassion and make choices that genuinely support our wellbeing.
